Matron Job Descriptions

Community Midwives are working to improve
health and quality of life for people with long term health.
They provide nursing and clinical care to people at home to
help them continue living in the community and avoid unplanned
hospital admissions.
As a lady of the community, would achieve a number of cases
of patients with a variety of complex health needs. Its tasks
include:
· Assess patient needs care and coordination of care plans
• Identify patients at risk of unnecessary admission to hospital
• Conduct a physical examination, diagnosis and treatment planning
· Monitoring of patients' conditions
· Patients With reference to other health professionals to
treat
· Teaching patients and family caregivers to detect changes
that could lead to your condition is worsening
· Organization of additional support such as home care or
respite care
· Provide information and health promotion advice to prevent
illness
· Maintain accurate and current patient records
· Keep in touch with community development initiatives.
